Hi, I’m Dr. Sapna Mukherjee. I grew up in Chicago and recently moved from the city to Glencoe with my husband, Dev. We have a 3 1⁄2 year old son and our daughter just turned one. Our sunny kitchen and family room definitely get a workout and we have been enjoying our backyard and the beautiful Glencoe beach as well!
I am on staff at Lurie Children’s (formerly Children’s Memorial Hospital) and Lake Forest Hospitals, and soon to be on staff at Evanston and Highland Park Hospitals.

Q. What services does Premium Care Pediatrics, P.C. (Premium Peds) provide?
Premium Peds is a full service pediatric practice offering well and sick child evaluation and treatment in the comfort of your home, at your convenience, Monday through Friday during the day. Visits with ill children will be scheduled the same-day or next business day. Generally, all appointments will be Monday through Friday, but weekend visits are possible if the issue can't wait until Monday, but doesn't need the ER. I will get to know your family and provide personalized care that is right for you and your child. I also offer free pre-natal visits for you to meet me.
Q. What ages of patients do you accept?
I care for children from birth through 18 years of age.
Q. How do I reach my pediatrician?
You can reach me on my cell phone 24 hours per day, 7 days a week. If I am seeing a patient when you call, leave a voice mail and I will return your call as soon as possible. For non-urgent calls that do not require an immediate response, I may return the call on the next business day or as soon as reasonably possible. For non-urgent matters, you can also reach me by text or email and I will respond as soon as possible.
Q. What do I do if my child becomes ill after hours?
Call me on my cell phone. Many issues can be handled over the phone without a trip to the emergency room. If you cannot reach me, the situation appears extremely serious or your child’s condition has drastically changed or deteriorated since you last spoke with me, it is always advisable to seek emergency care at the hospital or by calling 911. I encourage parents to use their judgment but it is my intention to be available to you as needed to help make decisions when you are unsure what to do!
Q: What do I do if I think my child needs immediate help?
In the event that your child's trauma or illness necessitates a visit to the emergency room I will ask that you take your child there. If you feel that calling me would delay needed care, go to the closest emergency room or call 911 if emergency transportation is needed. Call me from there while your child is getting needed care. Please do not delay care while waiting for me to return your call if the situation is urgent!
Q. Do you accept insurance?
In order to provide the highest quality of care in a personalized manner at your home, Premium Peds has adopted a fee-for-service model.
This means that I am not enrolled with any insurance providers and will not bill any insurance for the services I provide to your children. This approach allows for unhurried visits and a customized approach to your family’s needs. However, traditional insurance still has an important role, covering some primary care costs, emergencies, and hospitalizations.
Many patients with PPO insurance receive reimbursements from their insurers for some of the services I provide (subject to the specific terms of each policy). Families may also be able to apply their Flexible Spending Account (FSA) and Health Savings Account (HSA) for services provided by Premium Peds.
Contact your insurance company to find out about reimbursement rates for out-of-network providers and specifics of your program. Premium Peds will provide documentation of our services, which you can submit to your health care plans.
Premium Peds cannot guarantee that you will be reimbursed for the services and items which we provide, but we will provide any documentation that is reasonably requested to assist you in obtaining reimbursement. Be sure to ask your payor how to best submit forms for reimbursement and what the process may be.
Q. What are the fees for being part of the practice?
There is an annual fee and the rates depend on whether you chose to pay quarterly or in a one-time annual payment. This fee gives you 24/7 access to me by phone, email and text. There is also a separate visit fee for each house-call. I will provide you with information that outlines in detail the financial arrangement for my services.
Q. What if I need to see a specialist?
I will refer patients to specialists and for laboratory and radiological services as needed. Patients can use their traditional insurance for such services according to the provisions of their plans. I will provide any necessary written referrals that may be required by your plans for such specialty and ancillary services.
Q. Who will help me when you are on vacation?
My goal is to be available to my patients 24 hours per day, 7 days per week. For the few times when I am out of town or otherwise unavailable, another qualified pediatrician will take your calls and care for your child under the same financial terms that apply to my care. I will provide information about my colleague to all patients, as well as details of any planned absences, as far in advance as possible.
Q. How do you handle payment for services?
Both the membership fee and house-call fees may be paid by personal check or credit card. We will retain a credit card on file which will be charged automatically either upon request or, if you have asked to pay by check, in the event the check is not timely received. The dates for which you owe payment (for annual or quarterly fees) will be described in the Patient Services Agreement and Financial Agreement that we will execute when you decide to become a patient of Premium Peds. Payment for items and services provided to your children in your home are due at the time when such services/items are provided. We will generate an insurance claim for you, which you can submit to your insurer for reimbursement for covered services. This form can also serve as proof of medical service for your FSA or HSA account.
Q. From what areas are you willing to see patients?
My services are limited to the north shore of Chicago at this time. The boundaries for my services will be the following: South: Willow Road, North: Half Day Road, East: Lake Michigan, West: Highway 41/Edens Expressway.
Q. How can the relationship with Premium Peds be terminated?
If you decide that you no longer wish to continue with Premium Peds, you are required to provide at least thirty (30) days’ notice. Premium Peds will also provide such notice to you. Termination will, for financial purposes, be deemed to occur on the last day of the month following the effective date of termination. This means if you tell me you wish to terminate on April 12, your contract with me will end May 12, and you will be refunded any payments you have made for June and beyond. Any amounts owed to you will be refunded within thirty (30) days. In the event you owe an amount to Premium Peds at the time of termination, you will be expected to pay such amounts in full through the date of termination and Premium Peds will have a right to charge the credit card on file.
